Spring Sports 2012- Sign Ups

Student-athletes may begin signing up in the nurse's office on February1st if they are interested in trying out for a Spring sport 20112. To be determined as eligible to try out for any sport, whether you are a returning or new player, you must do the following:

  • Sign up on the list in the nurses office for the sport you are interested in playing. The lists for each sport are posted in the lobby of the nurse's Office. ***(Summer note: Students may come in to sign up for a Fall Sport all summer. The sign up list for sports is located in the lobby of the nurse's office, which is accessible all summer.)
  • If a current physical is needed, the Health Assessment Record form - (revised 04/2010, 3 pages), can be picked up in the lobby of the nurse's office. This form is to be completed by both the parent and the physician and returned to the nurse prior to the student-athletes participation in the sport. Please do not return this form to the coach.
  • Pick up in the nurse's office, complete, and return the Athletic Emergency/Permission Form at least one week prior to the start date. Please do not return this form to the coach. (**New :This form needs to be completed only once per year per student-athlete, not every season.)
  • If you are required to take the ImPACT test you may sign up for that in the main office, call 453-2741 X212 or email: Mrs. Scranton to schedule an appointment. ImPACT test dates are March 1st, 6th & 7th @ 2:30pm in room 50.

Forms should be dropped off in the nurse's office or in the main office before 2:30pm. They can also be mailed to:

The nurse determines your eligibility from a medical aspect and posts the sport lists that show whether the student-athlete is in compliance with all the required medical elements. This list will be given to the coach as a record of eligible student-athletes to try-out. All forms must be turned in to the nurse by noon in order to participate that day. (**See note below.**) Students who turn in forms to coaches will not be allowed to participate until the nurse can check the forms. If all elements are not completed the student-athlete will not be allowed to try-out until they are done. Please read the information page on the Medical Aspects of our sports program for more detailed information. All required medical elements are explained in detail on this page.

**Note: For the Fall season only, forms must be turned in to the nurse by noon the day before the season starts.
