PHYSICAL RESTRICTIONS:
A
reminder from the health office that should a temporary
or permanent condition prohibit your son or daughters
participation in Physical Education, you must provide
a physicians note documenting the condition and
physical restrictions. This verification should be filed
with the school nurse. If the condition is temporary,
the physician should provide a date by which the physical
restriction may be lifted and the student may resume regular
activities, including PE.
Should
your childs condition require he/she use an elevator
during the school day. Elevator keys are available from
the nurse. A ten-dollar deposit is required. Checks, payable
to Adams Middle School, will be returned when
the key is no longer needed and is returned to the nurse.
Connecticut
Education Statute 10-204a:
Legislation published March 1, 2000, has amended Connecticut
Education Statute 10-204a and now mandates immunization
against Hepatitis B (HBV), Varicella (chicken pox), and
measles, mumps, and rubella (MMR) prior to entry into
7th grade. Prior to entry into 8th grade, the Hepatitis
B series must be completed. In addition, Guilford Public
Schools require children to provide evidence of passing
a physical exam prior to entry into 7th grade.
A
form for documenting these immunizations and health requirements
is available in the nurse's office. If you do not have
insurance or if your insurance does not cover immunizations,
we recommend calling the VNA (458-4200). Based on your
call, the VNA will order the vaccine and offer it at minimal
cost.
The
health/immunization forms should be completed and signed
by your physician upon receiving the immunizations. The
form should be returned to the Health Office at Adams
Middle School .
