To Enroll:

The forms required for registration to Guilford High School are available in the Guidance Office. These forms include:

A Record Transfer Request Form

Health Assessment Record Form

Registration Information

Pupil Emergency Information Forms

Along with these forms the following are required by GHS in order to complete the registration process:

A copy of your child's Birth Certificate or a valid Passport

Proof of residency  ( 2 forms) example:  valid driver's license, utility bills, statement from major credit card, etc. showing where you live in Guilford.

Please be aware that medical, birth certificate, proof of residency and academic records as well as all of the forms listed above must be received by GHS prior to your child's enrollment.  Once we have complete registration information a meeting will be scheduled with your child's School Counselor.

 

To Withdraw:

The first step to withdraw your child is to notify his / her School Counselor.  Your child will be given a Release of Information Form and a Withdrawal Form which must be signed by a parent and returned to the Counselor.
