Medical Aspects of GHS Athletics Program

For each student-athlete to participate in an athletic program at GHS the following criteria must be up to date:

  • A current physical examination form must be on file with the school nurse in order to participate. The form required is titled Health Assessment Record - revised 4/2010 (3 pages). It is available at GHS in the main office or the nurses office or online. See Sports Physical Examination Policy for the complete physical examination requirement.

  • Prior to the first practice each candidate must submit to the school nurse a completed parental permission/emergency form. This form is also available at GHS in the main office and the nurses office or online. (**New:This form needs to be completed only once per year per student-athlete, not every season.)

  • ImPACT (Immediate Post Concussion Assessment and Cognitive Testing) is required of each student-athlete in order to participate in our athletic program.  Athletes playing certain sports will need to repeat the test their junior year. It will be used as a baseline assessment by our trainer and the student’s family doctor to determine when it is safe to return to play following a head injury.  The testiing appointment time is approxiamately 1 hour and must be taken on a school computer.  Students must have a computer authorization number to take the test.  Appointments to take the test must be made with the athletic secretary.
