Student Registration Information

Kindergarten Registration will take place on Friday, February 24th at each of Guilford's four elementary schools from 9:30 to 11 a.m. and 2 to 3 p.m.

- It is important to bring a completed registration form (download below) with you when you register your child.

- Please note that students registering for Kindergarten must be five years old by December 31st. The student’s original birth certificate, birth certificate with a raised seal, or valid passport,  must be presented at the time of registration.

- Kindergarten physicals completed within 12 to 15 months of the start of the school year are acceptable provided they are on the three-page blue State Health Assessment form (HAR-3). If the blue form is completed at the time of Kindergarten Registration on February 24th, please bring it with you. If you cannot provide the form at registration, it must be submitted by April 30th. If you cannot submit the form by April 30th, please contact the school nurse at your child's school as soon as possible. Health Records and Physical Exam Requirements

General Information:

New student registrations are accepted at the respective school your child would attend during the school year. Placement in an elementary school is determined by the family’s street address. If you are unsure as to which elementary school to register your child at, please call (203) 453-8200.

  There have been recent regulatory changes to school entry immunization requirements. These new requirements, which bring the Connecticut requirements into agreement with national recommendations, will be effective August 1, 2011.  All students effected will need documentation of up-to-date immunizations, submitted to their school nurse, prior to the start of the 2011-2012 school year.  Please consult your child's physician, to be sure they have received all required immunizations.
